Insurances, Use of Health Benefits, and Self-Pay Fees
In-Network Status: Dr. Zayco currently participates in-network with Tricare, VA, and Medicare. Veterans interested in seeing Dr. Zayco may be eligible to utilize VA benefits to see Dr. Zayco privately.
- If you have Tricare, you may have a $150 deductible, and your co-pay may be $0 or $31 depending on your plan. FYI Tricare covers tele-health services.
- If you have Medicare, after your $198 annual deductible, your co-pay is roughly $27 per session. If you have a Medicare Advantage plan that is managed by a commercial insurance company (such as United Medicare Advantage), your insurance is considered out-of-network with Dr. Zayco and you will be charged the self-pay fee.
- If you have the VA benefit, you have no out-of-pocket expense. You will need a current Mental Health Consult from your VA mental health provider (such as a VA psychiatrist, mental health therapist) in order to have psychotherapy sessions approved to see someone in the community.
Out-of-Network Status: Dr. Zayco is currently of out-of-network with Blue Cross, United Healthcare, Aetna, GEHA, Cigna, and Medicaid. If you meet with Dr. Zayco, you will be charged the self-pay fee at the time of service. (Special note to Blue Cross State Employee Plan individuals, such as UNF professors and employees: The Blue Cross State Employee Plan waives the out-of-network deductible for services, so you will only be responsible for 40% of self-pay fees.) As a courtesy, Dr. Zayco may submit your claims to your health insurance towards your out-of-network benefits. Dr. Zayco can explore with you whether your insurance has out-of-network benefits, which could offset the cost of receiving psychological services. Dr. Zayco may also provide you a super-bill to submit to your health insurance toward out-of-network benefit or to receive reimbursement from a medical or health savings account.
$160 for an hour-long diagnostic evaluation
$160 for a 55-minute individual psychotherapy
$185 for a 75-minute couple therapy session
